How do I remove or delete orders, results, and vaccines that were entered in error?

Context

Sometimes orders, test results, or vaccines may be entered incorrectly in a patient's chart - either in the wrong patient or with incorrect information. You may need to remove these entries to maintain accurate patient records and prevent billing issues.

Answer

For In-House Test Results and Screenings

If you've entered results for tests like strep tests, hearing screens, vision tests, or other in-house screenings in the wrong patient or with incorrect information:

  1. Go to the completed test result in the patient's chart

  2. Click on the Results icon to open the results side panel

  3. Select "Edit" on the result

  4. Mark the order as "Contraindicated for patient" (this is the most commonly used option for erroneous entries)

  5. Save the changes

This will revoke the order and remove the results from the patient's chart. If the provider has already signed the visit note, you should communicate with them to addend the note so the final documentation reflects the correct information.

For Vaccine Orders and Administration

For pended vaccine orders: You can cancel the order under "Orders and Diagnoses" before it's administered.

For administered vaccines: In the Vaccine Administration section of the visit note, click the "Undo" button next to the vaccine. This will remove the vaccine from the patient record and add the dose back to inventory.

Important: Do NOT revoke the pended vaccine order after a vaccine has been administered. Revoking the order will remove it from billing but will NOT remove it from the patient's immunization record. Always use the "Undo" button in the Vaccine Administration section to properly remove an administered vaccine from both billing and the immunization record.

For vaccine entries that can't be undone: If you see vaccine entries marked with "X" that won't delete, select option 4 or 5 (not options 1-3) to avoid formal documentation of vaccine refusal in the visit notes. Note that there is currently no "accidentally ordered" option - use "vaccine contraindicated" as the closest alternative when undoing erroneous entries.

For External Lab Results

If lab results were attached to the wrong order or patient, you cannot unattach them directly. However, you can:

  1. Find the completed order with the incorrect results

  2. Click on the Results icon

  3. Select "Edit"

  4. Upload the correct file/report that should be associated with that order

For Orders That Cannot Be Self-Removed

Some situations require vendor assistance:

  • Orders that have been accidentally marked as complete without results

  • VaxCare-integrated vaccines that were scanned but not actually administered

  • Complex document attachments with multiple patient results

  • Orders that are stuck in completed status

For these cases, contact support with the patient MRN and specific details about what needs to be removed.

Important Notes

  • Marking items as "contraindicated" removes them from charts without showing formal refusal documentation

  • Revoking orders also removes associated billing charges (unless billing was already submitted)

  • If a visit note is already signed, the provider should addend it after making corrections

  • For audit purposes, some revoked items may still show in tracking but won't appear in final visit notes
